Simply follow the guidelines to mail the package to the local social security office and receive your new Social Security card in two weeks!
What type of name change is allowed by the Administration?
You must inform the SSA immediately after a legal name change. The most common events for a name change are marriage and divorce. For those cases, you need to submit the original Marriage certificate or Divorce decree. If your name is changed for other reasons, you need to provide the original court order or Certificate of Naturalization showing your previous and new name.
What documents do I need to replace my Social Security card?
In order to change your name on your Social Security card, you need to prove your identity, your age, and your new name. You can mail one of the following combinations:
- U.S Passport and the name change document
- U.S State ID and the name change document
- U.S Driver's License and the name change document - U.S Military Card and the name change document
- School ID Card (showing your date of birth and a recent photograph) and the name change document - Government ID Card (showing your date of birth and a recent photograph) and the name change document
Important Note: the name change document can be:
1) Marriage Certificate, 2) Divorce Devree, 3) Court Order or 4) Certificate of Neutralization.
All documents submitted must be original. No photocopies are accepted. The Marriage Certificate and Divorce Decree must be Certified Copies.
Your ID and supporting documents will be returned in a separate mail since the agencies which validate your application and process your new card are not at the same location.
Tool Tips: If you need a Certified Copy of your Marriage Certificate or Divorce Decree, please use the Marriage Certificate Request on Vital Records Online.
How long do I need to have a Social Security card name change?
The social security administration requires two weeks to process your application from the date of reception. Depending on the number of requests and the local office workload, this processing time may vary.
If you are providing a court order and the administration has to perform an extra verification, this process may be extended by 1 to 4 weeks.
